即使不去医疗机构,也可利用电脑或智能手机的摄像功能等接受医疗服务的机制已建立。例如,通过连接互联网的“网络摄像头”,医生与患者进行对话;为行动不便的高龄者拍摄身体状况,通过平板电脑的摄像头让家庭医生查看并提供建议;在工作或外出时身体不适,通过智能手机向家庭医生传达症状进行诊疗——这样的情况都可以实现。

由于高龄、居住在偏远地区或工作时间等原因,确实存在就医困难的人群。其中一种对应措施是,将分开的医生与患者通过信息通信设备连接进行的“远程医疗”。不过,目前使用电脑或智能手机的远程医疗,主要是用于已就诊过的医疗机构进行“复诊”,而不是首次就诊。也就是说,主要对象是病情稳定的慢性病患者。另一方面,预计在不久的将来,将可以从全国的专科医生中选择想要咨询的医生进行智能手机咨询,或通过智能手机先询问症状是否需要立即就医的服务将实现实用化。

医療機関に行かなくても、パソコンやスマートホンのカメラ機能などを利用して医療を受ける仕組みが構築されています。今回は、スマホを用いた「遠隔診療」についてお話しします。

医療機関に行かなくても、パソコンやスマートホンのカメラ機能などを利用して医療を受ける仕組みが構築されています。例えば、インターネットに接続した「ウェブカメラ」を通して医師と患者が対話する。通院困難な高齢者の容態をタブレットのカメラで撮影して、かかりつけ医に見てもらってアドバイスを受ける。仕事先・外出先で体調が悪くなった時に、かかりつけの医師にスマホで症状を伝えて診察してもらう――といったことが考えられます。
高齢のため、あるいは過疎地に住んでいるため、さらには勤務時間などの理由で医療機関への通院が困難な人は確実に存在します。その対策の1つが、互いに離れたところにいる医師と患者を情報通信機器でつないで行う「遠隔診療」です。ただし現在のところ、パソコンやスマホを用いた遠隔診療は、まったく初めての医療機関の診察を受ける(初診)のではなく、一度かかったことのある医療機関で「再診」してもらう位置づけが主体のようです。つまり、慢性疾患で病状が安定している患者が主たる対象になると見られます。一方で、近い将来は、全国の専門医から自分が相談したい医師を選んでスマホで相談したり、すぐに医療機関に行った方がよい症状なのかをまずスマホで問い合わせたりするサービスが実用化される見通しです。

A system has been established to receive medical care using a computer or smartphone camera function without visiting a medical facility. For example, doctors and patients can communicate through a "webcam" connected to the Internet. The condition of elderly patients who have difficulty visiting clinics can be filmed using a tablet camera and shown to their primary care physician for advice. If one feels unwell while at work or outside, they can report their symptoms via smartphone to their primary care doctor for a consultation.

There are certainly people who find it difficult to visit medical facilities due to old age, living in depopulated areas, or work schedules. One countermeasure is "telemedicine," which connects doctors and patients who are physically apart through information and communication devices. However, at present, telemedicine using computers or smartphones is primarily positioned for "follow-up visits" to medical facilities that patients have already visited, rather than completely new consultations (initial visits). In other words, it is mainly intended for patients with stable chronic conditions. On the other hand, in the near future, services are expected to be realized where patients can choose the doctor they want to consult with nationwide via smartphone, or first inquire via smartphone whether their symptoms require an in-person visit.
